Incident reporting is a key part of making Ontario a safe place to work, live and play. Reporting an incident protects the public and helps industries prevent future issues. For regulated industries, reporting an incident is the law.
Report an Incident
The Technical Standards and Safety Authority (TSSA) must be advised of all reportable incidents by calling 1-877-682-TSSA (8772) and choosing option 1 to reach the Spills Action Centre (open 24 hours a day, 7 days a week).
Please call 9-1-1 for emergencies that require immediate assistance from the police, fire department or ambulance.
TSSA proactively collects and analyzes data to assess risks and investigates incidents to determine root causes and inform corrective actions. Reporting near-miss incidents and property damage, in addition to incidents with injury or accidents, provides us with a comprehensive set of data to understand risk and improve safety.

Incident Reporting by Industry
Amusement Devices (AD)
To report an incident involving an amusement device, please refer to the AD Incident Reporting Guidelines and complete the Amusement Device Incident Reporting Form.
Elevating Devices (ED)
To report an incident involving an elevating device, please refer to the ED Incident Reporting Guidelines or Incidents Involving Floods, and complete the ED Incident Reporting Form.
Passenger Ropeway (Ski Lifts)
To report an incident involving a ski lift, please refer to TSSA’s Ski Lift Incident Reporting Guidelines and complete the Passenger Ropeways (Ski Lifts) Incident Reporting Form.
Boilers and Pressure Vessels (BPV)
TSSA conducts incident investigations at the site of boilers, pressure vessels, pressure piping or fitting regulated under the Technical Standards and Safety Act, 2000’s Ontario Regulation 220/01: Boilers and Pressure Vessels. We also provide support to other regulatory agencies when needed. Fuels

Operating Engineers (OE)
To report an *accident or incident, call 1-877-682-TSSA (8772) and choose option 1 to reach the Spills Action Centre (open 24 hours a day, 7 days a week). To report an incident involving boilers and pressure vessels, please refer to the reporting guidelines in the BPV Incident Reporting Advisory.
