This is a clarification to the safety alert on Hollister-Whitney GT11/31 Machines issued on September 8, 2026, after a recent incident in Toronto involving the failure of an elevator traction sheave shaft. The emergency brake functioned as intended, safely stopping the elevator. No injuries were reported.
Improper reassembly of the machine was the cause of shaft misalignment and subsequent traction wheel shaft failure. The alignment of the traction wheel shaft is critical to the long-term performance of the machine. If a GT-Series machine has been disassembled, the subsequent reassembly must follow the process documented in GT-Series product bulletins, installation manuals and videos to ensure the shaft is aligned properly.
The manufacturer has instructed contractors to immediately remove from service any units with missing shims under the outboard stand and to contact their technical support for further guidance.
